Failure to Ensure PRN Psychotropic Medications Have a Stop Date

Pontchartrain Health Care CenterMandeville, Louisiana Survey Completed on 05-08-2024

Summary

The facility failed to ensure residents' drug regimens were free from unnecessary psychotropic medications for two residents. Resident #19 was admitted with diagnoses including Anxiety Disorders, Hallucinations, and Senile Degeneration of the Brain. The clinical record revealed an active physician order for Lorazepam 2mg/ml oral concentration, to be administered every 4 hours PRN for anxiety/agitation until death, without a documented stop date. Similarly, Resident #51, admitted with Bipolar Disorder and Schizoaffective disorder, had an active physician order for Lorazepam 2mg/ml oral concentration every 4 hours PRN for anxiety/agitation until death, also without a documented stop date. Interviews with the hospice nurse and hospice company's administrator confirmed that the Lorazepam medication is a PRN standing order for every hospice resident and does not have a stop date. The Director of Nursing (S2DON) and the Administrator (S1ADM) confirmed the oversight in not ensuring PRN psychotropic medications had a stop date and were limited to 14 days as required. The deficiency was identified during a review of the clinical records and interviews with the hospice nurse, hospice company's administrator, and facility staff. The facility's failure to adhere to regulations regarding PRN orders for psychotropic medications resulted in the continued use of Lorazepam without a stop date for both residents. This oversight was acknowledged by the Director of Nursing, who admitted to overlooking the requirement for a stop date on PRN psychotropic medications, and the Administrator, who confirmed the necessity of a stop date and the 14-day limit for such orders.

A resident with dementia and anxiety was prescribed Seroquel without appropriate clinical indications or documentation of attempts at gradual dose reduction. Despite the care plan's emphasis on monitoring and reducing psychoactive medication, no behavioral issues were observed, and the medication was not reduced or discontinued. Interviews confirmed the medication was used for unapproved indications, and the facility's policy on unnecessary drugs was not followed.

F0758 F758: Implement gradual dose reductions(GDR) and non-pharmacological interventions, unless contraindicated, prior to initiating or instead of continuing psychotropic medication; and PRN orders for psychotropic medications are only used when the medication is necessary and PRN use is limited.
Short Summary

The facility failed to address pharmacy recommendations for GDRs and non-pharmacological interventions for several residents, including a cognitively intact resident and a resident with cognitive impairment. Recommendations for reducing Buspirone and Fluvoxamine were disagreed upon without proper documentation of education or reasons for refusal. Another resident with severe cognitive impairment was receiving multiple psychotropic medications without specific justifications for disagreeing with GDRs. Additionally, an AIMS assessment was not completed upon initiation of Seroquel for a resident with moderately impaired cognition, contrary to facility policy.

Short Summary

The facility failed to complete AIMS assessments for two residents on antipsychotic medications, despite recommendations and physician orders. One resident, severely cognitively impaired and on hospice, was prescribed Risperdal and Ativan PRN without a stop date, leading to administration beyond the intended period.
