Summary

The facility failed to revise and update Care Plans to include and address high-risk medications and their side effects for two residents. Resident #17, who has diagnoses of diabetes mellitus and hypertension, was prescribed Insulin Glargine-Lixisenatide starting from 7/12/23. Despite the medication being administered consistently, the Care Plan lacked information regarding the usage and side effects of insulin injections. This oversight was identified through a review of the Minimum Data Set (MDS) assessment and the Medication Administration Record (MAR), which showed that the resident had no cognitive impairment and was taking diabetic medication and insulin injections daily during the look-back period.
