Summary

The facility failed to provide sufficient behavioral health care for a resident with documented behaviors, resulting in the resident eloping from the facility unwitnessed. The resident, who had severe cognitive impairment and used a wheelchair for mobilization, was upset about not being able to obtain cigarettes and exited the facility courtyard through an unlocked gate. The incident was not documented in the resident's progress notes, and the resident's care plan was not updated following the elopement.
