The facility's policies on charting, documentation, and hydration were not consistently adhered to, leading to significant deficiencies in the care provided to the residents. The lack of proper documentation and failure to follow care plans contributed to the residents' weight loss and dehydration, highlighting systemic issues within the facility's care practices.
