Observations showed Resident #27 using the Lap Buddy and wanderguard daily, and staff interviews confirmed that these restraints were in place due to the resident's history of wandering and poor safety awareness. However, there was no documentation of other interventions attempted or the necessity of continued use of these restraints. The facility's policy on the use of restraints, revised in April 2017, mandates that restraints should only be used for the safety and well-being of the resident after other alternatives have been tried unsuccessfully. The policy also requires a written order from a physician, consent from the resident or their representative, and ongoing re-evaluation of the need for restraints. The facility failed to adhere to these guidelines for both Resident #1 and Resident #27, resulting in the inappropriate use of physical restraints without proper evaluation, documentation, or consent.
