Additionally, the staffing information was not updated in a timely manner, as evidenced by the posting not being changed to reflect the current date on multiple occasions. Interviews with facility staff, including the Assistant Administrator, Staffing Coordinator, and ADON, indicated a lack of understanding and communication regarding the proper posting of staffing information. The Staffing Coordinator stated she was only instructed to post the projected hours for the day and was unaware of the requirement to include the total number of each discipline working per shift. The ADON confirmed that the detailed breakdown of staff by discipline was kept in a binder at each nurses' station, which was not accessible to residents or visitors. The Administrator expected the daily staffing to include the census and be broken down by shift, but this was not being done consistently. The facility's failure to post the required staffing information in a prominent and accessible location, as well as the lack of timely updates, had the potential to affect all 229 residents residing in the facility. The observations and interviews highlighted a systemic issue with the facility's compliance with its own policy and state regulations regarding the posting of direct care staffing numbers.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
