The facility failed to ensure food safety and sanitary requirements were met in the kitchen, leading to multiple deficiencies. Observations revealed that cutting boards and kitchen equipment were in unsanitary conditions, with cutting boards heavily marred and kitchen utensils such as a frying pan, slotted spoon, ice cream scooper, and lime squeezer found with encrusted grease, melted handles, and dried food particles. Additionally, food items were not discarded by their best-by dates, and temperatures of food items were not checked prior to preparation or distribution, with shredded cheddar cheese and fresh green salad found within the danger zone for bacterial growth.
