These deficiencies highlight significant lapses in the facility's infection prevention and control program, as well as in the training and execution of basic care procedures. The lack of proper documentation, adherence to testing protocols, and clear guidelines for perineal care contributed to these failures, potentially compromising the health and safety of the residents.
