Summary

The facility failed to revise and review the care plan for one resident, leading to an inaccurate Minimum Data Set (MDS) that did not indicate the resident was on hospice care. This series of oversights and miscommunications led to the resident not having an accurate care plan that reflected his hospice care needs.
