Summary

The facility failed to ensure resident assessments were completed within the required 7 to 14 days and transmitted to the CMS system for a resident who had passed away. The lack of a permanent MDS Coordinator and reliance on remote staff contributed to the oversight and failure to meet regulatory requirements for timely and accurate resident assessments.
