Interviews with the Social Services Discharge Planner, Nursing Supervisor, and Director of Nursing revealed that the facility's process involved notifying the Ombudsman on the day of discharge, contrary to the facility's policy which mandates that the Ombudsman be notified at the same time the resident is informed. The facility's policy requires a 30-day advance written notice of discharge, or as soon as practicable if the resident has not resided in the facility for 30 days. The failure to adhere to this policy resulted in the residents and the Ombudsman not being properly informed in a timely manner.
