Resident #38 experienced multiple incidents where harmful items were not removed from her room, leading to self-harm attempts. Resident #110 was also transferred without a gait belt, resulting in a laceration to the back of his head. Additionally, Resident #35 was transferred using a Hoyer lift by a single CNA, which led to a bruise on her left forearm. These incidents highlight the facility's failure to adhere to safe transfer protocols and ensure that staff were adequately trained and equipped to perform transfers safely, leading to multiple injuries among residents.
