The staff admitted to being aware of the incidents but failed to initiate formal investigations or report the allegations as required. The administration, including the Administrator, acknowledged the breakdown in the system and the failure to address the issues effectively. The Administrator admitted that the facility did not have adequate measures in place to prevent such incidents from slipping through the cracks, highlighting a significant deficiency in the facility's oversight and management of abuse allegations.
