Summary

The facility failed to timely identify, assess, and treat a pressure sore for Resident 24. The resident, who had a history of hypertension, type II diabetes, and congestive heart failure, was found to have an open area on the right buttock. Despite the discovery, there was no documented evidence that a thorough assessment was completed by a registered nurse (RN) or that timely treatment orders were obtained and implemented.
