Summary

The facility's abuse policy mandates immediate reporting and protective actions in cases of abuse, but these procedures were not followed. The LPN did not send the nurse aide off the unit or report the incident to the Director of Nursing (DON) or Nursing Home Administrator (NHA) immediately. This failure to act allowed the nurse aide to re-enter the resident's room and continue the abuse. The NHA later reviewed the incident with the LPN and confirmed that the nurse aide should have been removed from the unit and not allowed to return to the resident's room. The deficiency was identified during a review of policies, clinical records, and staff interviews. The facility's failure to ensure timely reporting and protection of the resident resulted in Immediate Jeopardy to the physical and mental safety of all residents. The incident highlighted a significant lapse in the facility's adherence to its abuse prevention policies, putting residents at risk of further harm.
