Resident #44, with diagnoses such as UTI and atrial fibrillation, was discharged to the hospital without a discharge summary. Resident #99, with chronic pain syndrome and diabetes, was discharged to home with home health services, and Resident #115, with CVA and diabetes, was also discharged to home with home health services, both without completed discharge summaries. Similarly, Resident 137, who had diagnoses including dysphagia, chronic kidney disease, and hypertension, was admitted for rehabilitation after a hospital stay and was discharged after being sent directly to the hospital following an outside appointment. As of March 26, 2025, no physician's summary was completed for Resident 137's stay from December 2, 2024, to January 10, 2025. The Nursing Home Administrator confirmed during an interview that they were unable to locate a physician discharge summary for Resident 137, reiterating the facility's expectation for such summaries to be completed.
