Additionally, the facility failed to ensure adequate supervision and safety measures for Resident #1, a paraplegic resident, during care. On February 21, 2024, Resident #1 fell out of bed while being bathed by a Nursing Assistant (NA #1), resulting in a superficial forehead laceration. The incident occurred despite the use of short upper side rails, which were later changed to half rails to aid in bed mobility and positioning. The Director of Nursing (DON) attributed the fall to improper body mechanics and positioning by the nursing assistant, and an in-service training on falls prevention was conducted for the staff. However, the facility's documentation and investigation into the incident were inadequate, as the DON did not document interviews with staff or conduct a thorough review of the events leading to the fall.
