Summary

The facility failed to maintain ongoing communication with the dialysis treatment center for a resident with end stage renal disease (ESRD) who required dialysis. The Unit Manager and Director of Nursing (DON) confirmed that the forms should have been completed and sent with the resident to the dialysis treatment center. The DON acknowledged that the facility did not have a process in place to ensure the completion of these forms. The Medical Director emphasized the importance of these forms for communication with the dialysis treatment center, and the Administrator confirmed that the nurses were responsible for completing them on dialysis days.
