Summary

The facility failed to ensure that the services provided met professional standards of quality, as evidenced by the handling of a fall incident involving a resident. On the night shift, a resident was found on the floormat and was picked up and put back into bed by an RN without reporting the fall or documenting an assessment. The resident was later found to have sustained an acute nondisplaced right femur intertrochanteric fracture, which was not reported until two days later during an interview with the RN involved.
