The facility failed to follow acceptable standards of clinical practice by not consistently documenting the administration of medications and treatments in the electronic Medication Administration Record (MAR) and Treatment Administration Record (TAR). Resident #2 had multiple instances where tube flushes were not documented in the MAR, despite having orders to flush the tube with water every six hours. The facility's policy on charting and documentation also required that all services provided to residents be documented in their medical records.
