Interviews with the Rehab Director and the Business Office Manager confirmed that the residents met their rehabilitation goals before discharge. However, the Business Office Manager admitted that the facility only issues NOMNC forms to residents who discharge from therapy services but remain in the facility, not to those who are discharged home. This practice led to the failure to provide the necessary NOMNC forms to the three residents, resulting in the identified deficiency.
