Despite these orders, the resident did not receive the recommended individual psychotherapy due to a communication error regarding insurance approval. Interviews with staff and review of medical records revealed that the facility's system for managing psychiatric services was flawed. The facility relied on an external service for psychiatric consultations, but failed to ensure that all recommended services were provided.
