The facility failed to prepare and serve food under sanitary conditions, as evidenced by several observations and interviews. During an initial kitchen tour, a large bag of shredded cheese was found opened and undated in the refrigerator, which was subsequently disposed of by the Certified Dietary Manager (CDM). Dietary Staff Z was unable to locate test strips for the dishwasher, and the CDM confirmed that the wrong strips had been ordered, necessitating the use of a three-compartment sink for dishwashing until the correct strips arrived. A follow-up kitchen tour revealed that the dishwasher machine was tested at 100 parts per million (ppm) by the CDM.
