Failure to Administer Prescribed Medications

Summary

The facility failed to ensure that Resident #1 received medications as per the physician's orders. Resident #1, who had diagnoses including atrial fibrillation and heart failure, tested positive for Influenza A and was prescribed Tamiflu 75 mg twice a day for 5 days. However, the resident received only 9 out of the 10 prescribed doses. Additionally, the resident was prescribed Albuterol nebulizer treatments three times a day for 5 days, but only received 8 out of the 15 doses due to the medication not being available. The resident also missed 8 doses of LiquaCel before the facility notified the provider and obtained a substitute order for Arginaid. The resident reported not receiving medications multiple times, including pain medication necessary for pain control. The facility's policy for ordering and receiving medications stated that medications should be routinely ordered and delivered in a timely manner, with an anticipated delivery time of 2 business days for refills. When there was a delay in dispensing medication, the pharmacy was supposed to notify the nursing staff, who would then document the reason for the delay and notify the physician or prescriber. However, this procedure was not followed, leading to the resident missing several doses of prescribed medications. The Director of Nursing (DON) acknowledged that residents should receive the number of medications ordered and that the staff should call the provider and get a hold order if a medication was not available. Despite this, the facility failed to ensure that Resident #1 received the prescribed medications in a timely manner, resulting in missed doses and potential adverse effects on the resident's health and well-being.
