Summary
The facility failed to ensure that nurse aides who had worked more than four months were trained and certified, and that those who had worked less than four months were enrolled in appropriate training. The DON and Administrator were unaware of the location of the necessary supplies for the training program, indicating a lack of organization and oversight in maintaining the training program's resources.
