Summary

The facility failed to protect a cognitively and visually impaired resident's right to be free from physical abuse from another resident with a known history of aggressive behavior. The aggressive resident, who had a history of agitation and violence, was not adequately monitored, and no care plan was in place to address his aggressive behavior. This led to an incident where the aggressive resident physically assaulted the impaired resident, causing significant injuries. The resident reported severe chest pain and other symptoms to the staff, but the staff did not take appropriate action to assess and address the pain. This lack of response led the resident to leave the facility and seek medical attention at a nearby hospital. The facility's failure to monitor and address the resident's pain and change in condition highlights a significant deficiency in the quality of care provided.
