The Executive Director admitted to not knowing who was responsible for reviewing the residents' EMRs to ensure accuracy. These deficiencies highlight significant lapses in the facility's adherence to professional standards for maintaining accurate medical records and ensuring proper care for residents. The issues with the splint application and the misfiled progress notes indicate a need for better training, clearer documentation practices, and more rigorous oversight of medical records to prevent such errors from occurring in the future.
