Summary

The facility failed to effectively use its resources to ensure medications were transferred accurately and completely to the facility's Electronic Medical Records (EMR), leading to a significant medication error for one resident. The resident, who had multiple serious diagnoses including atherosclerosis, embolism, and hypertension, was prescribed Keflex for an infection by his Infectious Disease physician. Upon readmission to the facility, the resident was given Keflex for five days before it was stopped, despite the physician's order for the medication to be continued for 180 days. The facility's Incident Log and Grievance Log did not initially reflect the medication error, and the resident's grievance was not addressed promptly. The Social Services Director (SSD) confirmed that the resident expressed concerns about the missed doses, but the investigation was still ongoing, and the grievance was not initiated when the issue was first identified. The DON admitted that the facility did not follow its policy for administering medications, which required medication errors to be documented, reported, and reviewed by the QAPI committee to inform process changes and additional staff training.
