The clinical record lacked documentation of temperature monitoring after the vaccine and hourly vital signs following a hypoxic event. The Regional Nurse confirmed the omission and stated that staff should follow the APRN's orders as directed. These failures in following physician orders and facility policies led to deficiencies in the care provided to the residents.
