Summary

The facility failed to ensure proper temperature control and sanitation in their food storage areas. Observations revealed that one of the refrigerators and one of the freezers were not within the proper temperature range. Additionally, food items in these storage units were not dated with open and expiration dates, which could lead to potential cross-contamination and foodborne illness. Dry goods on the shelves lacked expiration dates, and open food packages were not properly closed. The facility also failed to maintain a homelike environment by not removing meal items from serving trays and did not ensure a sanitary environment, as staff were observed reaching through the serving window from the dining side, and appliances were not clean, with food items left uncovered and walls and floors in unsanitary conditions. Specific incidents included a housekeeper reaching through the serving window, uncovered bowls of cereal with a fly landing in one, and a plastic bag of cane sugar left open on the counter. A can opener was found with a blackish broken substance around the blade, and empty boxes were stored on the floor in front of the janitor's closet. The walls and floors had a black substance buildup, and a storage container with breadcrumbs did not have an expiration date and had a scoop left inside. Various food items in the freezers and refrigerator were found without open or expiration dates, and some were expired. The freezer in the dry storage room showed a temperature of 16 degrees, and the ciabatta rolls inside were not frozen solid. The facility's policies on cleaning food preparation appliances, general food preparation and handling, and dry storage areas were not followed. The policies required that small appliances be cleaned and sanitized after each use, food items be covered for storage, leftovers be used within three days or discarded, and dry storage areas be kept clean and organized. The facility failed to adhere to these policies, leading to multiple deficiencies in food safety and sanitation practices.
