The Administrator also stated there was no notification, such as an email or error message, from CMS about questions or errors in the submission. The CFO stated the facility did not learn the file was rejected until a letter was received in the mail from CMS on 04/22/2026, and that a later attempt to resubmit the data was too late for Quarter 4 reporting.
