Health deficiencies cited at Helmwood Healthcare during CMS and state inspections, most recent first.
The facility failed to develop and implement written policies and procedures to prevent abuse, neglect, exploitation, and misappropriation of resident property. Record review showed OIG Abuse Registry checks were not completed for 2 newly hired Dietary Aides, and HR stated she only ran the check if an employee had a license number. The DON and Administrator stated HR was responsible for the required pre-employment checks and that the checks were important to keep residents and staff safe.
A resident with dementia and polyneuropathy missed five consecutive bedtime doses of Gabapentin because the prescription had run out and a new provider order was not obtained in time. MAR and progress notes showed the medication was unavailable, with staff documenting waiting on pharmacy delivery and later waiting for a new script. The DON said staff should have escalated the issue after the first missed dose, and the resident reported not sleeping during the period without the medication.

Summary
The facility failed to develop and implement written policies and procedures that prohibit and prevent abuse, neglect, exploitation of residents, and misappropriation of residents' property. Review of the facility policy, Abuse, Neglect, and Exploitation, revised 04/28/2026, stated the facility would develop and implement written policies and procedures that prohibit and prevent abuse, neglect, and exploitation of residents and misappropriation of resident property, and that potential employees would be screened for a history of abuse, neglect, exploitation, or misappropriation of resident property. Record review showed the facility did not complete OIG Abuse Registry checks for 2 of 8 sampled newly hired employees. Dietary Aide 1 was hired on 02/25/2026 and Dietary Aide 2 was hired on 03/20/2026, but there was no documentation that either employee was screened through the OIG Abuse Registry. During interviews, HR stated she completed pre-employment checks but did not run the OIG Abuse Registry check unless the employee had a license number, and if they did not have a license number she did not run their name through the KBN website. The DON and Administrator stated HR was responsible for the required pre-employment checks and both said the checks were important to keep residents and staff safe.

Penalty
Summary
The facility failed to ensure that one of three residents, R49, was free from a significant medication error when Gabapentin 300 mg ordered at bedtime for polyneuropathy was not administered for five consecutive nightly doses because the medication was not available. R49 was admitted with diagnoses including unspecified dementia and polyneuropathy, and the MDS showed a BIMS score of 11, indicating moderate cognitive impairment. The MAR showed no documented administration of Gabapentin from 05/01/2026 through 05/05/2026, with entries indicating the medication was not available. Progress notes documented that RN2 was waiting on delivery from the pharmacy and later waiting for a script from the MD after the provider was notified of the need. The record showed the last reorder date for Gabapentin was 04/28/2025, and the pharmacy packing slip reflected the medication was shipped on 05/05/2026 and received on 05/06/2026. The contracted pharmacist stated the prescription had run out and a new provider prescription was needed, and the new prescription was received on 05/05/2026. The DON stated she learned of the missed medications on 05/04/2026 after running a report and said staff should have notified her after the first missed dose; she also stated LPN4 should have followed up with the pharmacy and provider when the medication remained unavailable. The resident stated she missed her scheduled doses for about four days and did not sleep during that time.
