Surveyors also found food debris, crumbs, and trash on the floor in dry storage and behind equipment during multiple kitchen observations, and the DM said some areas were difficult to clean.

The facility failed to store, prepare, distribute, and serve food in a sanitary manner in the main kitchen. During a continuous observation of the lunch meal, a cook assembled resident meal plates while wearing gloves and repeatedly handled ready-to-eat foods without changing gloves or performing hand hygiene. The cook removed turkey from hot holding, cut it on a cutting board, and plated it for residents. She later removed her gloves and put on a new pair without washing her hands, then continued food preparation with the same gloves. The observation also showed the cook handling utensils and food items in a manner that did not maintain sanitation. She picked up a spoon by the bowl rather than the handle, placed it into a pot, and stirred. She continued using the same gloves while cutting turkey, transferring diced turkey to plates, handling potatoes with a scoop, and moving between tasks. She also removed gloves and did not wash her hands before donning new gloves, and she placed used gloves on the back preparation table instead of in the trash can. Later, she handled sandwiches on a cutting board, cut them in half, plated them, and then removed gloves without washing her hands. The kitchen was also observed to be unsanitary during two tours. Food debris and trash were found on the floor in dry storage behind the deep freezer, including bags of potato chips, crackers, dried pasta, crumbs, and two slices of bread behind a stainless steel table. The same conditions were seen on a later observation. The dietary manager stated staff used a cleaning schedule and swept and mopped daily, but also said it was difficult to clean tight spaces, under the shelf, and behind the freezer because of access issues.
