Summary

The facility failed to ensure a safe transfer for a resident with chronic obstructive pulmonary disease, hypertension, difficulty walking, unspecified dementia, and depression when two CNAs transferred him from a wheelchair to the toilet without using a gait belt. The resident’s MDS showed a BIMS score of 3, indicating severe cognitive impairment, and he required partial/moderate assistance with toilet transfers. His care plan identified an ADL self-care performance deficit related to cognitive impairment and weakness, with interventions calling for maximum assistance for sit-to-stand, chair-to-chair, toilet, and tub/shower transfers. Both CNAs stated the resident was a two-person assist and that a gait belt should have been used; the DON and Administrator also stated a gait belt should be used for transfers. The facility policy stated residents who require assistance in transferring will be transferred using a gait/transfer belt or with a lift.
