Failure to Care Plan Multiple Pressure Ulcers

Summary

The facility failed to develop a comprehensive person-centered care plan for Resident #74 that addressed her pressure ulcers. The resident was admitted with diagnoses including a displaced intertrochanteric fracture of the right femur, diverticulitis, a stage 4 pressure ulcer of the sacrum, a stage 3 pressure ulcer of the right buttock, a non-pressure ulcer of the right thigh, cognitive communication deficit, bladder neck obstruction, dementia, and need for assistance. Her admission MDS showed an indwelling catheter, bowel incontinence, and unhealed pressure ulcers/injuries, including one stage 3 and one stage 4 pressure ulcer present on admission and one surgical wound. The care plan dated 4/13/26 identified that the resident was at risk for further skin breakdown related to decreased mobility and incontinence, with interventions such as weekly skin assessments, notifying the MD for deteriorating ulcers, a pressure relieving mattress, and assistance with turning and repositioning. However, the actual pressure ulcers were not included on the care plan. Progress notes and wound documentation later identified multiple wounds, including a non-pressure ulcer of the right thigh, stage 3 pressure ulcers of the buttocks, a stage 4 pressure ulcer of the sacrum, and later bilateral heel DTIs that progressed to unstageable DTIs. The wound care orders in the record addressed multiple wound locations and treatments, including the right leg, right hip, right and left buttocks, sacrum, and both heels. The facility policy required a comprehensive person-centered care plan with measurable objectives and timeframes, developed within 7 days after the comprehensive MDS assessment and revised after each comprehensive and quarterly MDS assessment. During interview, the MDS Consultant stated that wounds should be care planned and that wound care should be added to the care plan to guide staff care, noting that there was a possibility interventions would not be put in place if wounds were not added to the care plan.
