Damaged Resident Room Walls Not Maintained in Good Repair
Summary
The facility failed to ensure two resident rooms were maintained in good repair and in a safe, clean, comfortable, and homelike condition. During observation on 07/13/2026, room 104A had friction marks on the door wall, torn and peeling sheet rock exposing the white chalky core, and a large hole in the sheet rock exposing the brown wood frame and insulation. Chalky white dust was observed on the floor. Room 112B had 4-5 linear abrasion grooves on the window wall, a large hole in the sheet rock exposing the brown wood frame and insulation, a visible gap where the outside wall could be seen, and chalky white dust and torn paper on the floor. During interview, CNA-J stated he had caused the initial damage in room 104A while adjusting the bed and said he verbally reported the wall damage to the Maintenance Man the day it occurred. He stated the damage worsened over time and that all staff could verbally report maintenance issues or enter them in the maintenance log. CNA-J also stated that not repairing the sheet rock could affect the facility's appearance and cleanliness. He said he did not know about the wall damage in room 112B. The Maintenance Supervisor stated he was responsible for repairs, checked the maintenance logs daily if possible, and expected repairs to be completed the day they were submitted unless parts were needed. He said he was aware of the damaged walls in rooms 104A and 112B since he had been hired, but the issues had not been logged in the maintenance binder. He said he had planned to repair them but had not gotten to those rooms yet because he was working on other areas that took priority. The Administrator stated repairs were expected to be completed the next day unless parts were needed and said she expected the facility and resident rooms to be homelike. Review of maintenance logs from 02/01/2026 through 07/13/2026 did not show reports for the wall damage in either room.
