Unsafe Food Handling and Hand Hygiene Practices

Arbor Post AcuteChico, California Survey Completed on 07-09-2026

Summary

Food safety and sanitation guidelines were not followed in the kitchen during food preparation and tray line activities. The facility’s Diet Order Tally Report indicated 118 residents received food prepared in the kitchen. Facility policies required hand washing before applying gloves, before donning a new pair, before handling food, and after touching contaminants such as trash cans or their lids. During observation, the Facility [NAME] was seen removing gloves and putting on new gloves on four separate occasions without washing his hands between glove changes. A Dietary Aide was also observed removing gloves, touching the trash can, and then donning a new pair of gloves without washing her hands between glove changes. Cross contamination was observed during food preparation. The Facility [NAME] prepared food on a countertop that appeared unclean, cluttered, and had food crumbs present. He returned cooked meat to the cutting board after touching the meat to a portion guide mounted on the wall, flipped the cutting board over and continued food preparation after the opposite side had contacted the countertop surface, returned from the dish machine to the food preparation area without proper hand hygiene, folded aluminum foil against his clothing before using it to cover vegetables, and touched the scale controls before handling cooked zucchini with the same gloved hand. During tray line, he was also observed touching his face and continuing to serve food without changing gloves or performing hand hygiene. In addition, six plastic plate covers were observed with thick white residue and rough, uncleanable surfaces, and the Dietary Manager confirmed they needed to be replaced.

The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
