Failure to Provide Individualized Activities for a Visually Impaired Resident

Almond View Care CenterWilliams, California Survey Completed on 07-23-2026

Summary

The facility failed to ensure that Resident 24 received an individualized activity program that accommodated the resident’s vision impairment and supported the resident’s interests, independence, and psychosocial well-being. The facility policy stated that activities were to be individualized and person-centered based on each resident’s assessment, interests, preferences, and abilities, with opportunities for residents to participate in planning activities and for activities to be adapted to individual choices and needs. Resident 24 was admitted with diagnoses including breast cancer, heart disease, abnormal mobility, glaucoma, and depression. The resident’s MDS indicated a BIMS score of 13 out of 15, showing no cognitive impairment, and that the resident was able to walk 150 feet with supervision or touch assistance. During observations, Resident 24 was seen alone in her room lying in bed. In interview, the resident stated that before admission she had been very active, but now spent most of her time alone in her room because limited vision prevented participation in activities. The resident reported that bingo was not enjoyable because she could not see the card, that her vision made it difficult to meet other residents, and that she felt lonely and surrounded by strangers. The resident stated that walking was her preferred activity, but she required staff assistance because she could not see, and she said she rarely got to go for walks, usually only about once a week. The Activity Director reviewed the July activity log and acknowledged that the resident was unable to walk independently in the hallway due to vision impairment, that there was no regularly scheduled walking program or individualized activity plan in place, and that walks were only occasionally offered. The Activity Director also acknowledged that audio books were available but had not been offered, and that the resident did not like dining room activities because she could not see or interact with other residents. The Administrator confirmed that the resident’s preferences had not been updated in the care plan after the resident’s preferred activity changed to walking, and that the interdisciplinary team had not been informed so the comprehensive care plan could be revised.
