The facility failed to implement the comprehensive care plan for one resident by not consistently providing oral care. The resident was admitted with diagnoses including metabolic encephalopathy, dysphagia, dementia, and adult failure to thrive. The Minimum Data Set dated 7/6/26 indicated the resident required maximal assistance with oral hygiene and had a BIMS score of 4, reflecting severe cognitive impairment. The MDS coordinator stated that the expectation was not met.
