Unsafe AMA Discharge Without a Safe Plan

Summary

The facility did not ensure a safe discharge for a resident who had multiple sclerosis, paraplegia, a suprapubic catheter, mild cognitive impairment, and required extensive assistance with transfers and activities of daily living. The resident used a motorized wheelchair and mechanical lift, had a care plan stating a goal to remain long term at the facility, and was taking medications including an anticoagulant, diuretic, and opioid. The facility had a sign-out process and staff told the resident to return by 11:59 PM, but there was no written policy in the record describing that a resident who did not return by that time would be discharged AMA. EMS was later called to assist the resident in the park, and the resident was taken to the hospital. The hospital record showed the resident was admitted with a principal diagnosis of multiple sclerosis and later discharged AMA. Interviews with the DON, NHA, RN, and SW confirmed there was no written policy documenting the 11:59 PM rule and no documentation that the resident had been informed of it in writing.
