Kitchen Food Safety and Sanitation Deficiencies

Whispering Pines Nursing And Rehab, LlcRipon, Wisconsin Survey Completed on 04-14-2026

Summary

Food was not consistently stored, prepared, and served in a safe and sanitary manner in the kitchen. During an initial tour, the Dietary Manager (DM-C) stated the facility followed the Federal Food Code and monitored cooking and holding temperatures, but the surveyor observed that holding temperatures were not obtained before, during, or after lunch service. Review of the facility’s Food Temperature Logs for the prior 30 days showed multiple incomplete entries for both cooking and holding temperatures across several weeks, and DM-C acknowledged that cooking and holding temperatures should be obtained for all meals and that the log entries were inconsistent. The surveyor also observed two toasters in the main kitchen that contained a significant amount of dried food debris and crumbs on both the interior and exterior. DM-C stated the toasters should be cleaned after each use or at least daily and acknowledged they should have been cleaned. The report cited the facility’s policy and the FDA Food Code requiring food-contact surfaces and cooking equipment to be kept clean and cleaned at least every 24 hours. In addition, the surveyor observed DM-C checking cooking temperatures with a probe thermometer without sanitizing it before use, after placing it on a prep counter with dried food debris, or after it contacted a cloth with food debris. DM-C repeated the temperature checks multiple times with the same thermometer without sanitizing it between uses. DM-C later confirmed the thermometer was not sanitized prior to use, after contact with the counter, or after contact with the used cloth, and verified that thermometers should be sanitized between uses.

Short Summary

Improper Thawing of Frozen Chicken: Staff thawed frozen breaded chicken in a 3-compartment sink without cold running water instead of using an approved defrosting method. A Cook said she removed the chicken from the freezer about an hour earlier, and the DM confirmed the chicken was thawed in the sink after staff forgot to remove it from refrigeration earlier. The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

D
F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Ice Machine and Water Filter Not Properly Cleaned: An observation found black and brown substance on the ice machine's water curtain guard and a dark brown water filter behind the machine. The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
