Medication Error and Unexplained Valbenazine Order
Summary
The facility did not ensure a resident was free from significant medication errors when the resident received another resident’s medications on 6/10/26. A nursing progress note documented that at 1800 the resident was mistakenly given medications meant for another resident, spit the medications onto clothing, and one acetaminophen tablet was not accounted for. The nurse practitioner was notified and blood pressure monitoring was ordered because a blood pressure medication had been included in the medications spit out. The nurse who administered the medications confirmed the error and stated the resident was monitored afterward, but no additional follow-up with the nurse regarding the error was identified. The resident had diagnoses including depression, hypertension, generalized anxiety disorder, hypothyroidism, orthostatic hypotension, type 2 diabetes mellitus, and unspecified dementia. The quarterly MDS documented a BIMS score of 2, indicating severe cognitive impairment, and the resident had an activated POA for healthcare decisions. The facility policy required staff to clarify medication orders that seemed unrelated to the resident’s diagnoses or condition before administration and to identify residents using two methods before giving medications. The resident was later prescribed Valbenazine Tosylate 60 mg beginning 6/16/26 and received 32 doses through 7/21/26. The record review did not locate a diagnosis of tardive dyskinesia or other documented indication for Valbenazine Tosylate. The psychoactive medication and behavior committee note did not document adverse side effects, monitoring, or interdisciplinary recommendations related to the medication, and staff interviews showed the ADON could not explain why the order was started. The life coach and pharmacist could not locate documentation supporting the medication, and the medication error document later stated the resident was started on Valbenazine due to a transcription error and that the medication had been ordered for another resident.
