Summary

The facility did not ensure appropriate Transmission Based Precautions were in place for 2 residents with pressure injuries. One resident had a stage 2 pressure injury to the left heel after a suspected deep tissue injury and wound debridement, but Enhanced Barrier Precautions were not implemented. During observation, a CNA and RN provided morning care, dressed the resident, and used a sit-to-stand device to transfer the resident to and from the toilet while wearing gloves but no gown. The resident had no EBP signage or PPE available, and the RN stated she thought a gown was only needed for wound care and did not think the resident was on EBP. The Infection Preventionist confirmed the resident should have been on EBP after the wound was debrided and agreed gown and gloves should be worn for direct or high-contact care activities such as transferring, toileting, and dressing. A second resident was admitted with a stage 2 pressure ulcer following a fall and femur fracture and was cognitively intact. The resident stated they had a pressure wound on the coccyx, and surveyors observed no EBP signage or PPE available in the room. The facility entered an order for Enhanced Barrier Precautions for the resident due to the pressure area, and a CNA later confirmed PPE was being used and that the reason given was the wound on the coccyx. The Infection Preventionist stated the resident was placed on EBP on 07/21/26, while the survey observation documented the absence of signage and PPE before that time.
