Food Storage and Kitchen Sanitation Deficiencies
Summary
The facility failed to ensure dry goods and refrigerated foods were properly stored, covered, and labeled with use-by dates for food safety tracking. During the kitchen observation, multiple items were stored on the floor, including a case of milk used to prop open a door, containers of salt, Glucerna, and Ensure, and two cases of frozen chicken breasts on the freezer floor. A cooling rack holding trays of cookies, chocolate brownies, and pecan pie was left completely uncovered in a high-traffic area where staff traveled in and out of the kitchen. The facility also failed to maintain food items with clear dates showing when they were prepared or when they should be used by. Multiple prepared foods and bulk storage items had only single dates or no dates at all, including cookie bags, juices, milk, thickened juice, salad mix, tuna, applesauce, pudding, cake, egg salad, opened lettuce bags, flour, rice, sugar, breadcrumbs, oats, and brown sugar. Staff stated they believed the markings were made dates, did not know what the dates meant, and said the facility did not place use-by dates on food bins or prepared food. Staff also stated the kitchen standard was to never use an item after it had been stored for more than three days, but direct observation showed multiple food items exceeded that threshold. The kitchen, dishwashing area, entrance, and rear exit were observed in an unclean and damaged condition. The food trap and drain under the dishwasher were filled with food and debris, the wall behind the dishwasher was coated with black dirt, grease, and grime, and the floor underneath was covered with garbage, black grease, and grime. The walk-in cooler area had damaged wall covering pulled away from the wall, exposing the underlying wall, studs, dirt, and black grime. The dish room door had deep scratches, exposed bare wood, and visible grease and grime. The entrance and rear exit area also had missing paint, exposed drill holes, an open utility access cover exposing pipes with rust, scattered garbage in the hallway, and a back door coated with grease and grime with broken weather stripping.
