Food and drink served at improper temperatures and with poor palatability

Sullivan Park Care CenterSpokane, Washington Survey Completed on 06-25-2026

Summary

The facility failed to provide food and drinks at proper temperatures and with palatable taste for 5 of 8 residents reviewed for food service. A policy titled Food Safety stated cold products are to reach temperatures no greater than 41 degrees Fahrenheit, and the facility’s schedule showed breakfast, lunch, and dinner service times. During interviews, Resident 4 stated warm food was cold and cold food was warm, and Resident 97 stated drinks were warm and ice cream was melted by the time meals were received. Resident 92 stated breakfast was delayed until 9:45 AM on one occasion, was cold, and that there was no organized system for tray distribution; the resident also described dry meat, tough orange chicken, mushy vegetables, and cold breakfast items on another occasion. Resident 56 stated food was served cold, meat was thin and overcooked, and meals were repetitive and lacked taste. Resident 48 stated the food tasted terrible, was cold when it should have been hot, and included overcooked items and lukewarm soup. Surveyor observations confirmed cold food and drink temperatures above the required range. On one test tray, milk measured 48.9 F, orange juice 57.9 F, and strawberry yogurt 44.2 F; on a second test tray, milk measured 52 F, apple juice 51.3 F, and peach yogurt 60.4 F. The dietary manager stated these temperatures were not within required parameters and confirmed cold food and drinks should be held at 41 F or lower. Observations also showed the Hall 100 food cart door was left open for 14 minutes during lunch tray delivery, and the Hall 200 food cart door was left open for 13 minutes during lunch tray delivery.

Short Summary

Food portions were not served per recipe directions when a cook prepared sandwiches, pureed items, cake, and side items by estimation instead of measured amounts.
