Dialysis Care, Fluid Restriction, and Medication Monitoring Not Followed
Summary
The facility failed to ensure a resident with end stage renal disease and dependence on renal dialysis was appropriately monitored and treated for blood pressure and fluid restrictions, and failed to accurately record dialysis communication into the medical record. The resident was cognitively intact and had an order for hydralazine every 8 hours as needed for systolic blood pressure over 170 or diastolic blood pressure over 110. Blood pressure readings showed elevated values on multiple dates, including 173/75, 174/80, 182/90, and 177/71, but the June and July MARs showed hydralazine was not administered on those dates. Dialysis communication forms showed the resident received a pneumococcal vaccine from dialysis, but the facility’s immunization record was not updated and instead documented that the resident had refused the vaccine after education. The resident also had a weight order for post-dialysis dry weights to be recorded after returning from dialysis, but the order did not list the goal dry weight established by dialysis. A later dialysis communication form showed a large amount of fluid removed during dialysis, with a pre-dialysis weight of 50.8 kg and a post-dialysis weight of 46.6 kg. The resident had a 1,000 mL fluid restriction with amounts assigned to nursing and dietary, but the MAR documented multiple days below the dietary amount and multiple days above the total restriction. During observation, the resident had hot water, bottled water, milk with cereal, and an unopened apple juice available at the bedside. Staff interviews showed the CNA was unsure of the restriction amount, dietary stated the resident should only get one cup of fluids in the morning, nursing did not know the dry weight goal, and the RCM/LPN acknowledged the facility missed the fluid restriction and that the resident should not have had both water and milk for breakfast or the apple juice.
