Failure to Initiate Bowel Protocol for Residents With Constipation
Summary
The facility failed to ensure bowel interventions were initiated for three residents who had orders and care plan interventions related to constipation management. Facility policy stated that when a resident had no or a small bowel movement documented for 48 hours, the licensed nurse was to assess the resident and determine whether the bowel protocol should be initiated, with Miralax, then Dulcolax suppository, then Fleet enema if needed. The deficiency involved Residents 82, 2, and 56, all of whom had diagnoses including dementia or cognitive impairment, and Resident 56 also had a care plan goal to have a normal bowel movement at least every third day. For Resident 82, the record showed extended periods without documented bowel movements, including 8 days and 6 days without a bowel movement in June 2026. Although PRN Dulcolax was documented on two dates and PRN Miralax was documented once, the record did not show bowel protocol interventions being initiated in response to the prolonged absence of bowel movements. The RCM/RN stated that the resident did not have bowel movements documented per the record and that the bowel protocol should have been initiated in addition to scheduled Miralax. The DON stated staff were expected to follow the bowel protocol. For Resident 2, the record showed 5 days without a bowel movement and another 4-day period without a bowel movement. PRN Miralax was given on day 4 of one no-bowel-movement period, but no PRN bowel medications were given on other days when the resident remained without a bowel movement. The RCM/RN confirmed no PRN bowel medications were given during the identified periods, and the DON stated the doctor’s orders should have been followed, including giving PRN bowel medications on the days identified. For Resident 56, the record showed no bowel movement for 5 days in June, 6 days in May, and 9 days in April 2026, with no PRN bowel medications administered during those periods. The RCM/RN confirmed the lack of documented bowel movements and the absence of PRN bowel medications, and stated this did not meet expectations for constipation management.
