The record stated the facility was not able to contact the resident’s representative, and there were no notes showing that a bed hold was offered or attempted for the unplanned discharge. During interview, the DON stated staff were expected to offer a bed hold at the time of discharge or to the resident’s representative, and after reviewing the record stated that did not happen but should have.
