Dental care follow-up and pain management not ensured

Rock Hill Health & RehabilitationBellingham, Washington Survey Completed on 06-01-2026

Summary

The facility failed to ensure the provision and follow-up of dental services for Resident 3, who was admitted following a stroke and had documented dental problems on the admission MDS and CAA. The MDS noted obvious and likely cavity or broken natural teeth, and the CAA documented some missing lower teeth in poor shape, an upcoming dental appointment, and a dysphagia diet. However, the care plan reviewed on 05/26/2026 contained only generic interventions to observe for complaints of pain or discomfort and refer to a dentist as indicated, with no individualized interventions or monitoring for the resident’s current dental problems or dental pain. During an interview and observation on 05/26/2026, Resident 3 stated they had sore teeth and gums, pointed to the right lower jaw area, and the mouth was observed to have missing and broken teeth with small white fragments visible, along with reddened and swollen gum tissue. The resident reported eating soft food and being careful brushing because the area was sore, and stated they had not seen anyone at the facility about the dental pain or dentition. Record review found no documentation of a dental visit since admission or any pending dental visit, although there was an order for topical mouth/throat gel for tooth pain and the MAR showed only one administration between 02/05/2026 and 05/28/2026. Staff Q stated the resident had gone to a dentist with their daughter about a week after admission, but the facility had no paperwork from that visit and did not know about any follow-up.

Failure to Obtain Timely Dental Extraction
D
F0791 F791: Provide or obtain dental services for each resident.
Short Summary

A resident with Parkinson’s disease and PVD had a fractured tooth that was evaluated by a mobile dental provider and the in-house dentist, who recommended extraction and prescribed antibiotics and pain medication. After physician approval and family consent, the extraction still was not completed because local oral surgeons would not accept Medicaid, the family was told to pay privately or change plans, and the record did not show coordination with a distant dental office or clear communication about transportation. The resident and family said transportation was never offered, and the resident remained without the recommended extraction for 8 months before death.

Short Summary

A resident with CKD and PVD, who was cognitively intact, had multiple non-restorable teeth and retained root tips with a plan for extractions so dentures could be made. Although medical clearance was obtained and staff noted the procedure would be scheduled, the record showed no completed extractions, no documentation explaining the delay, and no evidence the procedure had been scheduled during the survey period. The resident reported intermittent tooth pain for months while waiting for the dental work.
