Unsafe supervision and elopement monitoring failures
Summary
The facility failed to maintain a safe resident environment free of accident hazards and failed to provide adequate supervision for residents identified as wandering or elopement risks. For Resident 8, who was admitted on hospice with diagnoses including cerebrovascular disease, dementia, and chronic kidney disease, the record showed severe cognitive impairment, impulsive unsafe self-transfers, repeated falls, wandering, grabbing staff and peers, and episodes of restlessness and agitation. The care plan identified the resident as a high fall risk and directed staff not to leave the resident alone in a wheelchair without supervision and to conduct frequent safety checks, yet the record documented 12 falls since admission and multiple observations of the resident moving about unsupervised. Resident 8 was observed entering other residents’ rooms, attempting to enter employee-only and therapy areas, trying to get into a locked utility room, and approaching residents and staff without adequate intervention to prevent unwanted touching. On one occasion, the resident self-propelled into a newly admitted resident and then into the surveyor while using a medication cart handle to move down the hall, and staff intervention did not prevent the contact. The incident log also showed an altercation with another resident, but the affected residents were not placed on alert charting and there were no progress notes about the altercation. The record further showed incidents of the resident crawling on the floor that were not entered in the incident log, and there was no assessment for injuries or incident investigation to determine the root cause. For Resident 38, the record showed prior elopement episodes and changing elopement risk assessments, including documentation that the resident was no longer an elopement risk despite a history of elopement and wandering. The physician order still documented a wander guard in place, but there was no supplemental documentation for nursing staff to check and document placement as required by facility policy. The elopement binder contained resident identification forms for some residents, but Resident 31, who was documented as an elopement risk with a wander guard in place, was not placed in the binder, and Resident 19 was documented as an elopement risk without a wander guard bracelet in place. Staff interviews confirmed that Resident 38 did not have a wander guard on at the time of observation and that the binder was not assigned to a specific person to update.
